Eligibility and Benefits Fax Back

To make your eligibility and benefits requests more user-friendly, when you call via the Interactive Voice Response (IVR) phone system, you will be given the option, following the quote, to receive a fax back confirmation of that quote.

The information contained in the fax is the same information provided to you via the automated phone system. For your convenience, this faxed information is your record of the eligibility and benefits quote. Here is an example of the information that will be returned.  Remember the system will only return the eligibility and benefits information you have specifically requested.
